What is a “Vaccination Certificate of COVID-19”(Vaccine Passport)?

A Vaccination Certificateof COVID-19 (hereafter: “Vaccination Certificate”) is issued to those who need a document proving that they had vaccinations in order to travel abroad or for other reasons. For the time being, this certificate is supposed to be used for overseas travel. The use of this certificate may ease restrictions such as epidemic control at immigration check-points.

Informationon Vaccination Certificate (Japaneseand English)

The Vaccination Certificate will mention the following: recipient name, date of birth,nationality, passport number, related vaccination information (date, number of doses, name of the vaccine manufacturer and distributor, product name and lot number, and country name where you received the vaccination) and so on.

 

Eligibility for an application to the City of Sendai

All of the following must apply:

  • You need proof of having been vaccinated for reasons such as expected travel overseas.
  • Your residence registration was in Sendai City at the time of having received the COVID-19 vaccination.
  • You used the vaccination tickets issued by the City of Sendai to receive vaccinations or you were a prioritized vaccination recipient such as a healthcare worker.

※ If youreceived either the first or second vaccination outside Sendai City due to movingin/out between the times of the vaccinations, you need to apply to each municipality for the certificate of both vaccinations.

(e.g.:If you moved to Sendai City and received the second vaccination after receiving the first one in city A,you need to apply for the Vaccination Certificate to both city A and the Cityof Sendai.)

 

How to apply

Mailing only. Please send the required documents to the office below by post.

Required documents

(Must have these items

※ Valid required documents are the “refugeetravel document,” “laissez-passer,” “travel documents issued by the country of your destination,” or the “re-entry visa.”

  • A return-addressed envelope【Please write recipient’s name and return address on an envelope and attach postage stamps. (An envelope size 120mm×235mm costs 84 yen.)】

※You need acopy of your identity verification documents if you do not have the items listed below.

  • A copy of coupons (vaccination tickets) ※ Aremaining part of the coupons after receiving vaccination, for example, the “Prevaccination Screening” part of the coupons.
  • A copy of certificate of vaccination, record of vaccination or prevaccination screening questionnaire ※If these documents are not submitted, issuance of the certificate may take time.)

When using the following items, please hide personal information with masking tape.

My Number card: “Individual Number”

Health insurance card: “Insurer number,” “Insurance number,” and the “QRcode (if printed).”

(Required items when a proxy applies.)

(Required item if your passport has original/alternative surname, or another name)

  • A copy of identity verification documents for applicant’s original/alternative surname, or another name

For issuance

The issued certificate will be sent by post using the return-addressed envelope which was enclosed at the time of application.  

Also,the certificate will be sent about one week after we receive your application.

However, if you do not have any proof of your vaccination record, it may take about one and a half month to two months as we need time for confirmation.

 

Note

  • The Vaccine Certificate cannot be issued when your passport is invalid.
  • Only one certificate can be issued per application.  
  • We would like you to apply only when you truly need the Vaccine Certificate due to inevitable reasons such as when overseas travel is expected. For domestic verification, you can use another type of vaccination certificate, which was given when receiving a vaccination, or a record of vaccination.
    ※The another vaccination certificate is issued at the vaccination venue where you received a vaccination. The vaccination certificate is a document with information regarding the vaccination you received. The form is enclosed in an envelope with vaccination tickets.
    ※A record of vaccination is issued to those who are medical workers and prioritized to received vaccination using vaccination tickets with the Prevaccination Screening Questionnaire.
