Consent of the recipient must be obtained prior to the administration of vaccines.
If you are currently undergoing treatment for other illnesses or have any concerns about receiving vaccination due to your physical condition, please consult with your family doctor in order to decide whether or not to receive the vaccination.
On the day of vaccination, individuals who has a resident record in Sendai City is eligible to be vaccinated at a hospital or vaccination venue within the City.
However, those with inevitable reasons such as long-term hospitalization or long-term stay at a care facility can receive the vaccination at a hospital or vaccination venue within the City, regardless of the location of their resident record.
Vaccinations are free of charge.
For those who have not received their COVID-19 vaccination yet, please receive your primary vaccination. Individuals who have received their primary dose are eligible to receive booster shots.
Individuals who are 6 month-old or older are eligible.
Since September 20, 2023, Omicron (XBB.1.5)–adapted vaccines have been used for primary vaccinations.

2 doses administered 3 weeks apart and a third 8 weeks after receiving the second dose. (3 doses in total)
Those who have received their primary vaccination between September 20, 2023 and March 31, 2024 are eligible for a single booster shot.
The vaccines used are those adapted for the Omicron (XBB.1.5) variant. Individuals can receive booster shots once 3 months have passed since their last vaccine.
Types of Vaccines
The Omicron (XBB.1.5)-adapted vaccine (Pfizer Inc. / Moderna)
※One could also opt to receive a booster shot using the Takeda (Novavax) monovalent vaccine. In that case, one can receive their booster shot once six months has passed since receiving their last vaccine .
※The Pfizer vaccine is the only one used for booster shots for 5 year-old children.
※The Moderna vaccine is intended for individuals 6 years old or older. The Moderna vaccine is the same as the one used for individuals 12 years old or older, however the dose amount is halved for those under the age of 12.

Individuals vaccinated with any of the following vaccines overseas can receive their next dose of vaccine in Japan.
Individuals vaccinated with vaccines that are NOT listed below will receive primary doses of vaccines in Japan.
※Individuals who were vaccinated with vaccines manufactured by Janssen Pharmaceutical overseas will be considered as having received two primary doses in Japan. Those who received vaccines manufactured by other companies will be considered as having received the same number of doses as one would have had in Japan.
Vaccination ticket is necessary for one to receive vaccinations.
For infants, vaccination ticket for the primary dose will be sent out when they are approximately 6 months old. Vaccination tickets for other individuals have already been sent out.
Vaccination tickets for booster doses will be sent out when one becomes eligible to receive them.
If you have an unused vaccination ticket for booster shots, a new ticket will NOT be sent out. Please use the vaccination ticket that you already have.

Vaccination is conducted as “Individual Vaccination,” which is administered by local medical institutions.
Please contact the medical institution administering vaccinations directly to make an appointment.

※Currently, “Mass Vaccination” administered at venues such as Shimin (Civic) Centers is NOT available.
On the day of vaccination, please ensure you are in good health prior to leaving for your appointment. You cannot receive a vaccination if you have a fever or is in poor health.
The vaccination ticket is an all-in-one form with the Prevaccination Screening Questionnaire. Filling in the Questionnaire before heading to the vaccination venue will help you save time during the reception procedure.
※Please fill in the Japanese version of the Prevaccination Screening Questionnaire while referring to the translated foreign language version available on the website of the Ministry of Health, Labour and Welfare (Link to external website).
E.g. Resident card, driver’s license, health insurance card, etc.
This form is used at the vaccination venue to prove that an individual has been vaccinated. It was sent out along with the vaccination ticket. For individuals receiving their booster dose, the vaccination certificate is printed in an all-in-one form with the vaccination ticket and Prevaccination Screening Questionnaire. DO NOT separate the forms and bring it with you to your vaccination appointment.
※The vaccination certificate serves as proof that you have been vaccinated, so please do not lose it.
※Only applicable to individuals between the ages of 6 months old to 11 years old
Upon receiving COVID-19 vaccinations, some individuals might experience side effects such as pain at injection site, feeling of tiredness, headache, and muscle pain.
In Japan, in order to correspond to side effects such as anaphylaxis, it is required for individuals who were vaccinated to stay at the vaccination venue for 15 to 30 minutes. If an anaphylactic reaction occurs, immediate treatment will be provided on site.

Upon receiving COVID-19 vaccinations, some individuals might sustain health problems as a result of experiencing side effects.
Although this is very rare, the possibility of it occurring is not nonexistent. To counter this risk, relief system is in place.
Please consult the Home Health Section of Ward Offices or the Public Health and Welfare Section of General Branches regarding the necessary procedures.
