Individual Number (My Number) Card Application Support

Guidance on how to fill out the Individual Number (“My Number”) Card Application form and ID photo format will be available at the Temporary Individual Number Card Center. 

  • Opens: April 12th (Mon.)
  • Operation hours: Weekdays from 10:30a.m. to 5:30p.m.
  • Venue: Temporary Individual Number Card Center (AER bldg. 5F)
  • What to bring: Application for issuance of the Individual Number Card (for those who already have the document), Personal ID documents (Driver’s license, passport, govt. issued National Health Insurance card, Physical Disability Handbook etc.)

 Financial Assistance for School Materials and Lunch Fees etc.

Parents and guardians of students attending municipal elementary or junior high schools or Sendai Seiryo Secondary School (lower secondary course only) who are finding it difficult to send their children to school due to financial difficulties, are eligible to apply for assistance.

  • Eligibility: Households with a yearly income below a certain level
    Households receiving a child-rearing allowance
    Households that no longer receive welfare allowance

Deadline for Filing Inhabitant Tax Declaration is April 15th (Thu.)

Please either come to the venue or file your declaration by post. To prevent the spread of the Novel Coronavirus we ask that Inhabitant Tax Declarations are filed by post when possible. Application forms can be downloaded from the City of Sendai website, or can also be created with the Inhabitant tax Declaration tax calculation/declaration creation corner(External link).

  • Location: Aoba Ward Office 9F Meeting Room, Sendai City Hall North Bldg. 5F, Inhabitant Tax Section
  • Time: 9:00a.m.- 11:00a.m., 1:00p.m. – 4:00p.m.
    *There will be no free income-tax declaration consultation sessions with tax accountants.
    *Depending on the congestion levels at the venue, the cut-off times may end sooner than the times listed.
    *If a tax declaration was filed on March 16th or later the initial municipal tax calculation may not be reflected in the declaration’s contents. In this case a revision notification will be issued in the following days so we ask that you pay the initially issued tax notification while your revision notification is delivered. The difference will be adjusted in the following tax payment period.

Don’t forget your Application for Citizen’s Health Checks

The deadline for the first application period for receiving a basic health check and cancer checks run by the City of Sendai is April 26th, 2021. The application guideline is distributed at Ward Offices and Shimin Centers. Applications can also be completed on the City of Sendai’s official website. Please check the application guide or inquire for detailed information.

Exemption or Postponement of National Pension Payments can be Applied Retroactively for up to Two Years and One Month Before the Month of Application

Those having trouble paying national pension premiums will be able to apply retroactively for exemptions, payment period extensions, and student payment exceptions for a period up of up to two years and one month before the month of application. If your application is approved, you will be granted an exemption from, or extension on, national pension premiums.

BCG Vaccinations

BCG Vaccination to be undertaken at each Ward Office and Miyagi General Branch Office. BCG vaccinations may be received outside your ward of residence. Please check the City of Sendai Website for dates and times of vaccinations or inquire for detailed information.

  • Eligibility: BCG vaccinations are given to children under one year of age.
    (The standard age for BCG vaccinations is five to seven months old.)
  • What to bring: Maternal and Child Health (MCH) Handbook, BCG vaccination coupon from the MCH Handbook voucher packet (please fill out and detach)
  • Please wait at least 28 days after your child receives a live vaccine (such as polio) or seven days after your child receives an inactive vaccine (such as DPT-IPV) for BCG vaccinations.
  • Please go directly to the vaccination site. (Reservations not required.)
