Disaster Victim Certificate / Disaster Victim Report Application

A Disaster Victim Certificate / Disaster Victim Report will be issued to those with a damaged house or building (apartment/company) resulting from the Tohoku Pacific Ocean Earthquake and the following tsunami. These documents are essential when applying for various disaster assistance services offered by the City of Sendai.

  • A Disaster Victim Certificate shows the degree of damage to houses and buildings and will be issued upon assessment.
  • A Disaster Victim Report shows that the applicant has filed a claim for damage to their house, building and personal property.
  • There are some disaster assistance services that only require a Disaster Victim Report and not a Disaster Victim Certificate. The Disaster Victim Report will be issued on the day of application. Please contact each organization that provides assistance services directly.

Note

Since the damage caused by the earthquake and tsunami was extensive, it may take some time to receive the Disaster Victim Certificate after submitting the application. We ask for your kind understanding on this matter. If you wish to start repairing the damage before an assessment inspector visits, please take pictures of the damage and keep them with a written estimate of repair costs or a detailed statement. You will be asked to provide them at a later date.

For businesses:

Those who are applying for any kind of disaster relief loans and need a Disaster Victim Certificate / Disaster Victim Report should prepare the following:

  1. Photos that show the damage (building or property, etc.)
  2. Residential area map that can prove the location of the building
  3. If you are corporate business, please bring a copy of the Certificate of Registered Information (*the certificate is only valid for 3 months)
  4. Other documents that prove the degree of damage.

Things to bring when applying:

Identification card (resident registration card、driver’s license、passport etc),

A letter of proxy is necessary when you ask someone outside of your family who lives at the same address to apply on your behalf.
